Dr. Howard Quackenbush established Delta Physical Therapy and Sports Medicine, P.C. in April, 2000. After outgrowing the location at the Delta Community Hospital in 2012, Delta Physical Therapy moved across the street to a new and larger facility allowing us to serve an even greater number of people in our community. 

In 2005, a second clinic was opened in Fillmore, UT. Located in the Fillmore Community Hospital, Fillmore Physical Therapy just relocated to a new state of the art facility.

We are a state-of-the-art Physical Therapy Clinic recognized in the community as THE place to go for a personal approach to health and wellness.

Our mission is to provide the HIGHEST quality of physical therapy and to be the FIRST referral choice for ALL in our community.
